The above are sample good faith estimates for uninsured and self-pay individuals with minor health problems. Each patient is unique, as are their presenting conditions and treatment. Actual pricing will be reviewed before performance, and payment in full due before service. We accept payment by cash, credit card, check, FSA, HSA, and HRA.
